You can also ask your physician to refer yourself or a loved one to Community Health Center. Either way, services will soon be coordinated between CHC and the doctor for the most comprehensive home care available in the 7 counties we serve. The majority of CHC services are overseen by a nursing supervisor, under your doctor's plan of care. CHC is like an extension of your physician's office, as they approve all care given to you by CHC.

CHC directly bills Medicare, Medicaid, or private insurance on your behalf. Home health care services that may be covered by Medicare and Medicaid include skilled nursing care; home health aides; rehabilitative physical, speech and occupational therapies; social work services; and medical equipment and supplies. Your condition keeps you from leaving home without help (such as using a wheelchair or walker, needing special transportation, or getting help from another person).

Registered Professional Nurse case managers directly supervise services provided and coordinate other services based on physician's orders, patient needs, and risk assessment. Someone who is high risk due to physical or cognitive impairment; complex medical needs; those who lack family support; at high risk for repeat hospitalizations.

read more › The NHTD program is a New York State Medicaid Waiver program that offers individuals with disabilities and seniors who are eligible for nursing home placement to receive services in their home and community. This option allows for a greater quality of care in the least restrictive setting possible. Seniors will receive appropriate homecare services permitting them to live in the most appropriate, least restrictive setting, which is usually their homes rather than an institution. The NHTD program is targeted at individuals either already in nursing homes and wanting to return to their community, or those being considered for nursing home placement but who would prefer to avoid institutional care and remain in their own homes.

Physical Therapy (PTs) provide services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries or disease. Treatment often includes exercise for patients who have been immobilized and lack flexibility, and range of motion before finally advancing to other exercises that improve strength, balance, coordination, and endurance.

A wound care nurse works with a patient's homecare team to monitor a variety of wounds and the healing process.
